Jenny O'Donnell

Director

Jenny O’Donnell is the founding director of Systemic Factors (formerly known as TOD Safety Engineering Limited). Jenny has worked in safety investigation for over 22 years and is an experienced human factors consultant with a background in systems engineering, training and lecturing. Jenny is a chartered engineer and has an MSc in Human Factors.

As a visiting fellow of Cranfield University, Jenny delivered investigation training to the Healthcare Safety Investigation Branch (HSIB) national investigators and was a course director for the first ten cohorts of maternity investigators. Jenny currently provides human factors support to HSIB on a consultancy basis.

Jenny is a visiting lecturer of City University delivering modules in Human Factors and Accident Investigation on their MSc Aviation Management Programme. She has also delivered human factors and safety training across several health and social care settings, in addition to a number of transport organisations. She is also a lecturer for Babylifeline investigation courses.

Jenny is registered with the Engineering Council, is a member of the International Society of Air Safety Investigators, the Royal Aeronautical Society and the Clinical Human Factors Group. She is also an advisor for the Before Arrival at Hospital and iPoorly studies. Jenny has been a patient member for NICE clinical guideline developments and she is a volunteer for the UK Sepsis Trust, running support groups for those affected by sepsis.
