Only a small proportion of incidents will progress to full Patient Safety Incident Investigation (PSII). This Patient Safety Review Course recognises that providers need to have staff trained to undertake less resource-intensive investigations that will deliver safety learning in a timely but rigorous way. Lower-level investigations need to be robust enough to feed the Patient Safety Incident Response Plan (PSIRP) and contribute positively to an open culture. Like the full PSII course, this is also a skill-based course.
Course Content
Sample course content includes:
- Immediate action review (in the event of a hazardous situation)
- Collaborative debriefing with staff and family
- Analysis including timelining/chronologies and SEIPS based ‘Care System Interactions and Outcomes Worksheet’
- Hazard-based risk assessments
- Systemic factors learning reviews:
- Examinations of case records/note for rapid review (e.g. SJR)
- Templated specialised reviews for known areas of harm (e.g. falls, pressure ulcers, VTE)
- Proactive learning reviews including:
- Appreciative and impartial inquiry
- Ethnographic observation and audits to understand work-as-done
- Management of ‘hazard hunt’ findings and adopting good practice
The full course is recommended to be 1.5 days but clients can choose from any of the topics above to be a one-day or half-day session.
This is a practical workshop where all delegates will be encouraged to interact, ask questions, and respond to class challenges. We do this via discussions and group exercises. To help us manage this, we cap delegate numbers to 20 per workshop.
Our courses are currently mostly delivered in an online format (e.g. via Microsoft Teams/Zoom) at times to suit your staff and can also be delivered in separate standalone modules as required.
